Option 1
For tax exemptions under 80G, you could address the cheque to Samavedana

In this case Samavedana is free to use the money for "ANY" hospital expenses, not only for Rishi
Hence you need to specifically give a standing instruction to Samavedana that the money needs to be used for "Hrishikesh Sarafdar" ONLY
This will assist your cause and all of our efforts

Samavedana uses this money for Rishi ONLY when Rishi is admitted to Sahyadri hospital - as a practice, they do not disburse this money for buying day to day medicines. This is also a big reason why we always recommend Option 2 to our donors

Option 2
You could send the money to Vishnu's account
We will disburse the money to hospital / Samavedana when in need
And acquire 80G receipts in the name of donor
These receipts will be sent to donors via post
If this money is required for out-of-hospital expenses, we will get regular medication receipts, not necessarily 80G
